By signing this roster, I am aware of certain risks and dangers associated while playing coed softball. I assume all risks by participating in the Stunt Softball League and hereby hold harmless the Stunt Softball League including but not limited to their executive board members, agents, representatives and assigns of any and all liability, actions, causes of action, debts, claims and demands of every kind and nature whatsoever which I now have or which may arise out of or in connection with participation in any activities with the Stunt Softball League. This agreement further states it operates as a release and assumption of risk for his/her heirs.

As team representative, I hereby acknowledge that all of the above information on this roster is correct and in no way falsified. I have read and understood all the Rules of the Stunt Softball League.
